All clients will need to fill out a waiver before their first class.
In consideration of being allowed to enter the yoga, fitness and art area and/or participate in any activity at Lolo’s Studio LLC, the undersigned, on his or her behalf, and on the behalf of the participant(s) identified below, acknowledges and agrees to the following conditions.
I acknowledge that I am the parent or legal guardian of the child(ren) and voluntarily authorize my child to participate in yoga, fitness and art with Lolo’s Studio LLC. I understand that there are inherit dangers associated with yoga, fitness and art, and recognize that any physical activity involves risks of serious bodily injury or death, including but not limited to temporary or permanent muscular and skeletal injury and paralysis, which may be caused by my own actions, or inactions, those of others participating in the activities, the conditions in which the activities take place, or the negligence of the participants named below; and that there may be other risks either not known to me or not readily foreseeable at this time. This could include, but not limited to twisting an ankle, broken bones, torn ligaments, or even death. This includes outside the building and all surrounding areas.
Physical activities with Lolo’s Studio LLC will include, but are not limited to: Running, jumping, stretching, twisting, pulling, pushing, headstands, handstands, push-ups and planks. Students may practice various inversions (including but not limited to headstands, backbends). Conditioning can be strenuous and demanding. Such activities can result in joint, bone and muscle stress and fatigue, shortness of breath, sweating and accelerated heart rate. Knowing these risks, I and my child(ren) assume all risks, whether foreseen or unforeseen, in connection with my child participation in these activities. Parent/Guardian authorizes these supervised activities.
Off-site Activities: Often Lolo’s Studio LLC participants will enjoy yoga, fitness and art activities off-site, including but not limited to at the park or in the neighborhood. Off-site events will involve the same or similar activities as referenced above. Condition of off-site areas and equipment is not known by Lolo’s Studio LLC, staff or contractors. Though Lolo’s Studio LLC will exercise due caution, we do not guarantee the safety of the off-site location and will not accept responsibility for injuries sustained during off-site activities. Parent/Guardian agree to hold Lolo’s Studio LLC it’s staff, and/or contractors harmless from accidents or injuries that occur during off-site activities.
Medical Release: I fully understand that the staff of Lolo’s Studio LLC are not physicians or medical practitioners of any kind. With that in mind, I hereby authorize and give my consent to Lolo’s Studio LLC including any of its instructors or other authorized contractors or employees to provide emergency medical care and to give authority to any emergency unit, hospital or doctor to render immediate aid as might be required for the treatment of the above named student in the event of any emergency either on the premises of Lolo’s Studio LLC or during the course of any activities involving the child(ren) as a Lolo’s Studio LLC participant.
Media Release: By attending Lolo’s Studio LLC activities, in the studio or off-site, I hereby grant to Lolo’s Studio LLC the absolute right and permission to copyright, publish, and use photographic portraits, pictures, audio, videos, audio taped or photographed by any means and grant full use of my child’s likeness, voice and words without compensation to me or the participant(s).
COVID-19:
I acknowledge the contagious nature of the Coronavirus/COVID-19.
I further acknowledge that Lolo’s Studio LLC cannot guarantee that I or my child will not become infected with the Coronavirus/Covid-19.
I attest that:
* I/my child is not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I do not believe that I/my child have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
I hereby release and agree to hold Lolo’s Studio LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of Lolo’s Studio LLC, or that may otherwise arise in any way in connection with any services received from Lolo’s Studio LLC. I understand that this release discharges Lolo’s Studio LLC from any liability or claim that I, my heirs, or any personal representatives may have against Lolo’s Studio LLC with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Lolo’s Studio LLC. This liability waiver and release extends to Lolo’s Studio LLC together with all owners, partners, and employees.
Lolo’s Studio LLC is not responsible for lost items or for items left behind.
I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN LOLO’S STUDIO LLC, AND ME, AND I HAVE SIGNED IT OF MY OWN FREE WILL.
